Sign Prior Approval form

Federal law requires IFDAPAC to obtain authorization from member corporations before soliciting the corporation’s executive and administrative personnel, stockholders, and their families. Only an authorized representative of the member corporation may authorize solicitations. Further, a member corporation may not allow solicitations by IFDAPAC if the member corporation has authorized solicitations by another trade association PAC within the same calendar year. On behalf of my company, I hereby authorize IFDAPAC to solicit me and any eligible individuals I designate regarding contributions, programs, special events and activities related to IFDAPAC during the calendar year(s) below indicated by my signature. My company has not authorized a solicitation by any other trade association for the same calendar year(s). I understand that IFDA will never solicit my employees for financial contributions without my knowledge and permission. I am entitled to share this information with the executives, administrative and professional personnel of my company if I so choose and, entirely at my own discretion.
